The Hugging Face Breach Has Been Attributed, in Part, to OpenAI's Organizational Culture

The culture was not breached. The culture was, however, identified as a contributing factor to a breach at a separate institution. This is a new category of incident. Adequate is creating a form for it. The form will require a signature from the culture.
